DENTAL INSURANCE

We currently offer dental insurance plans from two companies, CompBenefits and Citizens Security Group, for Kentucky State Employees. All of these plans are payroll deductible, on the 30th of the month, with the effective date being the first of the month following the deduction.

Clicking the CompBenefits logo will take you directly to their web site where you can access the dental enrollment forms online. You may “right click” on the enrollment form link and choose “Save Target As” to save the form to your computer.  CompBenefits will allow you to fill out the form online and then click “File”, “Save As”, to save the completed form to your computer.

 Clicking the Citizens Security Group logo will take you directly to their web site where you can access the two dental enrollment forms online. You may “right click” on the enrollment form link and choose “Save Target As” to save the form to your computer.  Then you must print out the form, and when you have completed it, fax it to Krol Ins at 502-875-3615.

We request that you name the file as “LastName_FirstName_MiddleInitialD”. (Example: Smith_John_JD123456789). When you type your name and date in the signature area, please add your employee ID number and this will be your electronic signature that you have completed the form.

In either case, you will need to save the document to your computer and then email it to us at krolinsurance@bellsouth.net , or fax it to us, for further processing. We will then process and send the completed form to your payroll officer and also to CompBenefits or Citizens Security as appropriate.

 VISION INSURANCE

We offer vision insurance plans from two companies, CompBenefits and Citizens Security Group, for Kentucky State Employees which is payroll deductable, on the 15th of the month.

Clicking the CompBenefits logo will take you directly to their web site where you can access the vision enrollment form online. You may “right click” on the enrollment form link and choose “Save Target As” to save the form to your computer. CompBenefits will allow you to fill out the form online and then click “File”, “Save As”, to save the completed form to your computer.

 Clicking the Citizens Security Group logo will take you directly to their web site where you can access the vision enrollment plan form online. You may “right click” on the enrollment form link and choose “Save Target As” to save the form to your computer.  Then you must print out the form, and when you have completed it, fax it to Krol Ins at 502-875-3615.

HEALTH INSURANCE

We offer Anthem and Humana health insurance plans for Kentucky State Employee dependents.

Clicking either of the Anthem or Humana logos will take you directly to their web sites where you can fill out an application online. Your application will be sent directly to the appropriate company for processing and Krol Insurance will receive notification of your submittal. Before open enrollment is closed, we will notify you of the company's decision. The policies will be effective 01 January 2008 for one (1) year and can be continued beyond one (1) year until canceled.

 These health policies are individual, and will be paid by bank draft or monthly billing.
