DENTAL INSURANCE
We currently offer dental insurance plans from two companies, Humana/CompBenefits and Citizens Security Group, for Kentucky State Employees. All of these plans are payroll deductible, on the 30th of the month, with the effective date being the first of the month following the deduction.

Clicking the Citizens Security Group logo or word will take you directly to their web site where you can access the two dental enrollment forms online. You may right click on the enrollment form link and choose Save Target As to save the form to your computer. Then you must print out the form, and when you have completed it, fax it to Krol Ins at 502-875-3615.
We request that you name the file as LastName_FirstName_MiddleInitial. (Example: Smith_John_JD123456789). When you type your name and date in the signature area, please add your employee ID number and this will be your electronic signature that you have completed the form.
In either case, you will need to save the document to your computer and then email it to us at krolinsurance_at_bellsouth_dot_net, or fax it to us, for further processing. We will then process and send the completed form to your payroll officer and also to Humana/CompBenefits or Citizens Security as appropriate.

VISION INSURANCE
We offer vision insurance plans from two companies, Humana/CompBenefits and Citizens Security Group, for Kentucky State Employees which is payroll deductible, on the 15th of the month.

HEALTH INSURANCE
We offer Anthem and Humana health insurance plans for Kentucky State Employee dependents.
If you wish to discuss obtaining a Humana policy, please call this office directly at 502-875-3477 to discuss your needs.

The policies will be effective the first of the month that you select. Please understand that this is a minimum (1) year contract. The policy can be continued beyond one (1) year until canceled.
These health policies are individual, and may be paid by bank draft or annual billing.
